Dear Colleagues,

We are inviting all the International Alumni of the Medical University of Gdańsk to participate in the International Alumni Mini Interview – IA Memories and Experience.

Please, provide your basic personal information and answer 5 interview questions in the form below. Then, please kindly choose to provide your consent to our processing of your information and your answers. We hope for your consent to publishing of your basic data and your answers to the interview questions both on this website (alumni.mug.edu.pl) and also in a PDF brochure, which we intend to produce and make publicly accessible at the websites of the Medical University of Gdańsk. The volume of the PDF brochure is limited, and therefore only 10-12 interviews will get selected for publishing in the PDF brochure, while all of the interviews will be available on the website, following your consent. We are asking you for your contact email address to be able to contact you if we want to verify your submission of the answers to the interview questions or consult you about the edited version of your answers, when such editing proves necessary to fit your answers to the space available in the PDF brochure.

Also, if you agree and wish to have your photo displayed next to your mini interview, please send us your badge-style photo in decent quality (JPG or PNG format, 3×4 cm) to iaro@gumed.edu.pl. In such case please copy the text below (Photo submission declaration) as the text of the email message:

GDPR INFORMATION CLAUSE

The controller of personal data processed for contacting MUG International Alumni is International Alumni Relations Officer Medical University of Gdańsk, Dr. Jacek Kaczmarek, ul. M. Skłodowskiej-Curie 3A, 80-210 Gdańsk, Poland, (hereinafter: “Controller”). Please be informed that:

  • Your personal data will be processed for purposes related to publishing your answers to the interview questions in a non-anonymous manner (with your personal information – names, year of graduation, speciality and country of residence – disclosed to public audience;
  • Your data will be processed pursuant to Article 6(1)(a) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (“GDPR”);
  • Personal data will be stored for as long as Controller’s actions in the area of international alumni relations are implemented;
  • You have the right to request from the Controller access to and rectification or erasure of your personal data or restriction of processing;
  • You are authorized to revoke your consent at any time, which will not affect the lawfulness of any processing performed based on that consent prior to its revocation;
  • You have the right to lodge a complaint with the President of the Office for Personal Data Protection if you consider that any of your personal data are processed in breach of GDPR;
  • Personal data are collected from the data subject directly;
  • Providing your data is voluntary, but necessary in order to execute publication of your interview answers and personal information;
  • Your personal data will be processed with the objective to make your answers to interview questions available in a non-anonymous manner to the public audience both at the website of the Medical University of Gdańsk and in a PDF brochure available from that website.
  • Your data will be added to an off-line database managed locally by the IARO MUG in a secure manner.
